Conditions for obtaining service

  1. An elevator should be available in the building if the center is situated within the floors of the building.
  2. The service should be specified (a technician for males and a technician for females).
  3. The center should be supervised by a specialized doctor.

Procedures

  1. Referring to the Health Institutions Licensing Department and filling the form of licensing a physical therapy center and attaching the official letter of the Ministry of Industry and Trade to state the purpose.
  2. Referring to the register of the Health Professions and Institutions Licensing Directorate to document the application and transferring the application to the director of the Health Professions and Institutions Licensing Directorate to review and transfer the application to the Health Institutions Licensing Department.
  3. Transferring the application with the required documents the Health Institutions Licensing Department.
  4. Providing the applicant with the approval official letter on registering the purpose.
  5. Referring to Health Institutions Licensing Department and providing it with the documents of commercial register and trade name.
  6. Studying the application by the committee of licensing physical therapy center and scheduling an appointment for field inspection and prepare a report about that.
  7. If the equipment of the physical therapy center complies with the regulation of licensing therapy centers.
  8. Payment of the fees in Finance Section.
  9. Preparing an official letter signed by the Minister with the licensing approval and work commencement. 

Required Documents

  1. An official letter from the Ministry of Industry and Trade stating the purpose.
  2. A copy of the title deed or lease contract.
  3. A copy of the organizational location plan.
  4. A copy of land plan.
  5. A copy of practicing professions certificate for technicians.
  6. A copy of commercial register and trade name.
  7. A copy of ID for the applicant.
  8. Written pledge by the specialized doctor to supervise the center.
